You choose a reward “Track” from those available to you, and as you progress along it you get rewards based on the track. This will often include things like gear/skins, as well as other loot associated with the track’s theme.
Some tracks are unlocked all the time, others only during parts of the year, and a few more (dungeons) can be unlocked for good by clearing the dungeon story mode. Some regional ones are unlocked by clearing other regional ones.

There used to be rank-based PvP rewards in the game, but they were removed in the feature pack of April 2014. Instead, PvP now yields PvE rewards.
Yes, it was an awful decision in every way imaginable with no redeeming aspect to it whatsoever, and no one really knows who came up with such a regressing and destructive idea, but that about summarizes ArenaNet’s decision making of 2014.
The only exclusive PvP rewards remaining are finishers (your PvP rank is now otherwise completely meaningless), and one set of armor (Glorious) which can be obtained via the respective reward track.
Numerous players hope that other previously PvP-exclusive armors (such as Tribal) will become available again (as they are currently no longer obtainable in the game), but as of yet, we have no word from ArenaNet on the matter.

Allow me to politely disagree with your assessment of the value of the decision. In fact, that change is the only reason I even touch the game mode, other than using the HotM as a shortcut to Lion’s Arch.
Am I a great player? No. I doubt most would even call me a good player in PvP. But I try, and that’s only because of the rewards change. I know I’m not the only one that was pulled in by it, too.
