The whole ladder is reset every week and servers are ordened by their new ratings.

Every server has a rating. WvW uses a modified version of the Glicko 2 system that is also used in chess. After each match, your server’s rating is recalculated based on your performance against your recent opponents. Doing well against a higher rated opponent increases your rating for example.
All servers in the same region (NA or EU) are ranked from highest rating to lowest rating. The top three servers are tier 1, next three are tier 2 and so on until tier 8 in NA and tier 9 in EU.
Your server will face the other two servers in the same tier. The highest of the three is the green server, next is the blue server, last is the red server.

Also, there has been a lot of transfers between servers lately, that has really affected the rankings. Some servers are ranked too high, while some are too low.
