This is an extremely simple question, but I couldn’t find the answer anywhere.

What happens when you’re in Lich Form Death Shroud mode and your Life Force reaches zero? Do you go back to the health you were at when you initiated? Or back to some percentage of max health?

Lich form does not interact with LF

You cant be in Lich Form (the elite skill) and death shroud at the same time.
If you mean lich form = death shroud then yes you will go back to your original health. You cannot be healed through death shroud.

oops, sorry folks. I incorrectly said lich form. Thanks, you answered my question!

Testing this would cost you just a few seconds. Activate Shroud and wait until your life force is gone.
Spoiler alert: your regular hp stays the same (unless you take damage).

You will go back to your original HP that you had when you entered DS, however if you get forced out of DS due to damage, any damage that your LF didn’t absorb will rollover.
