You might consider some things.
You don’t have to do anything once your are in plague form, except tap the cast you want once. Plague autocasts that spell from that point in time on. So hitting 2 once will make every single tick blind.
Healing in shroud. sigh Everytime a necromancer mentions being healed in the death shroud, a kitten dies and the dark lord Cthulu weeps about the living souls he can’t get. You. Can. Not. Be. Healed. In. Deathshroud. Period. You are not being healed, you will not be healed and you never were healed in deathshroud. Every time you tap your shroud and the ele next to you tries to save you by healbursting. Yes. It does NOTHING. Except unholy sanctuary. Which is a bad trait.
Don’t run flesh worm. Flesh worm is a horrible skill. Flesh worm is not the stun breaker you want, nor is it the blast finisher we need. Flesh worm is an abysmal joke of a skill. Spectral walk provides you with much more mobility, and can’t be killed. Spectral armor provides you with a lot more tankyness, and can’t be killed.
BUT. I liked your usage of locust swarm and lift transfer a lot. Seems to be a nice combo I’ll try in the future. And you can rez in shroud now. I think. If they didn’t remove it.

I have to disagree on the Flesh Wurm. Flesh Wurm is awesome. Be it for trolling (Wurm Stomp), to get places you usually couldn’t or GTFO fast. Spectral walk just doesn’t cut it for that. Because you need to trigger it before the fight and then finish the fight quickly enough to be able to use TP. Also many people run both, anyway.

Many high tier necro PvP’ers would disagree with you on the wurm subject >.>
I would disagree on Unholy Sanctuary being a bad trait. It’s quite good, you just need to build around it (like grandmaster traits really should be). Running Zealot’s gear does a lot to make it good.
However, in PvP, I’ve found you can usually get away just fine with Zerker or Assasin’s amulet if you go for protection uptime. That’s a more selfish build, though, and this isn’t bad for a support Necro build. That said, I personally would swap Spiteful Vigor out for Staff mastery.

Flesh wurm seems bad when you aren’t prepared to use it or when you don’t know what it’s truly capable of. But once you learn it’s an entirely different experience on necro.

I wish I knew how to use it. It seems good for porting around Khylo, but a lot of the time it seems very useless to me. I’ve gotten to the point where in Spvp I run spectral wall more often than Wurm, allows me to get annoying melee off of me and sometimes to get a rez off, but again thats proably due to my inexperience.

There is a video floating around of all the places you can port. I think I it’s from a thief perspective but it still applies for wurm. The only map it doesn’t feel quite as dominant (to me) is Niflhel because it doesn’t have as dramatic of escapes, but khylo, legacy and temple all have great spots for ports.
I’m still learning to use it, and when I get lazy and don’t pre-place it, it always seems bad in that moment – the cast time is too long to make an escape if it isn’t prepared. But when you’re using your ports in the right moments, it’s really a lot of fun.
Flesh Wurm actually gives you even more flexibility because it doesn’t need to have LoS and is 1200 range. So for example if you place it between home and mid on Foefire, you can teleport from the very edge of the middle point all the way to Flesh Wurm.
