Pretty basic Shortbow, Sword/Torch Survival character with 0/10/30/30/0 trait setup.

Would it be best to go with a full condition approach (Rabid, Dire, or some mix of these) or with a Hybrid approach (Carrion, Rampager, or a mix) to take advantage of the two grandmaster minors and the 10 points in Skirmishing?

rabid is what you need, full armor and accessories, giver for weapons. Rabid is not full condition approach, its also give presicion which is much more important for your build than power. Take Prickly Pear as food and toxic oil as utility(100% bleed druration and 70% crit chance with fury). 2 birds and muddy terrain ifyou dont mind the slow running.
change oakearslave to off-hand training its very good with bonfire. If you stick with oakheart, nature’s bounty> nature protaction.

Use sigils of earth and blood to draw directly against you opponents health pool. Blood will be very powerful with rabid amulet against direct damage because of high toughness. This kind of build is great for slaughtering high armor opponents.

I’ll go ahead and recommend Hybrid :P

Rampagers weapons + armour with knights trinkets.

Your conditions will do around 29% less damage than with full rabid, but you will have 1800 power allowing for some actual direct damage. Throw Battle Sigils on each weapon set and you’ll be laughing with both Direct and Condition damage, not to mention them also helping your pet a bit.

You lose a little armour as well, but 2860 is more than enough IMO.
