At least from the limited testing i’ve done.

Sick’em now doesn’t break if you use other abilities, it also doesn’t do that stupid pause anymore if you use it to start a fight (thus canceling the Sick’Em)

Testing it some more resulted in the pause happening, Trying to figure out why it was working before hand and now has started the pause.

Pet f2’s will still break it though, I think the pet views the F2’s as his own skills so it cancels out Sick’Em (which would be considered like an F2 in its on right)

you can still do the F2 first then Sick’Em though.

But things like Quickness/Signet of the Hunt don’t break it.

Drakes opening attacks (the Blast Finisher) will also cancel Sick’Em soon as they get to the target, so don’t bother with drakes. (This part might be related to the Pause before hand instead, I haven’t figured out why it works when you first log into the game but quickly changes to requiring you to attack the target first before using Sick’Em so the pause doesn’t happen)

There’s this hidden property to freshly swapped pets.
They’re overly eager, and will take weaponskill presses with a target selected as a hidden F1 command. You could technically wave a melee weapon miles away from a target, and a freshly swapped pet will take that as a cue to tear it’s throat out. This effect lasts until you press Return for the first time, and comes back everytime you swap it in.

Maybe what’s happening is the hidden F1 in the eager state is giving the pet the necessary mindset to sally forth without interruption when told to Sic ‘Em, but once it’s lost it’s eagerness the hidden F1 isn’t there anymore and the pause happens?
