So, perhaps someone can answer this for me — occasionally when using cloak and dagger, the attack will land and hit but I’ll also get an ‘immune’ message (this is strictly related to PvE – haven’t really seen it as much or at all in PvP) and I won’t go stealth. What mechanic causes this? Anyone?

When you stealth and exit it by performing a hostile action, you gain a “Revealed” buff. It prevents you from entering stealth again for a short while.

This is to prevent certain skills like Shadow Refuge, from constantly putting you into stealth allowing you to spam your #1 stealth skill or gaining 2 initiative every time from the Infusion of Shadow trait in the Shadow Arts tree.
