Withdraw: change idea
I have never had that issue, that’s where map/terrain awareness comes into play, this would make it very clunky, it doesn’t heal for much and as it sits now allows for avoiding bursts from other professions and mobs with the evade happening when it initiates, other wise the heal would more likely be wasted
It is a problem sometimes – if I’m immobilized and locked into one direction I can’t let my char roll towards me. Same with SB3 – I think it has been different back in the day and the char always moved towards the player. Until they “improved” SB3 in one patch.
I agree with BlaqueFyre. To avoid falling you can still weapon swap to cancel the roll. You don’t get the heal but in dungeon that should be ok.
As mentioned, there’s very little reason to need it. Quite frankly, I see it as a good thing as it rewards players who are smart and play into the weakness of an individual player by cornering the thief into a location which they can’t simply just escape from. This is one of the distinctions between a mediocre or good opponent from a great one.
If only Daredevil carried this similar risk.

It is really down to practice. I don’t think I ever had any troubles in PvE (open world) and if I know what’s up in front (last boss thaumanova fractal e.g.), I’m switching my heal skill. In fact in that very fractal I’m using the most uncommon thief skills and weapons anyway: P/P and SB, Thieves Guild and Hide in Shadows (if I don’t forget that which I often do).
Once upon a time Anet was very eager that we switch skills and don’t just set them and call it a day for the rest of our time in this game. And actually, they were/are right with that.
If you’re a daredevil, which I assume (I’m not) then you don’t need that extra dodge from withdraw.

Again this is a Terrain and Character awareness issue, I use Withdraw in every game mode in all maps, how you fix it is to make your characters back face the direction you want to go if you do that every time you will go the proper direction, I have never fallen to my death when using this heal in Hot maps, WvW where cliffs are abundant or anywhere else. The only time you will get stuck is if you are in the middle of a Shadowstep animation or animation similar to Vault or Death Blossom.
Honestly; the only difficult thing about HoT is map completion and not because I fall to my death (oh I have a glider, nifty), but because I have to get to the bottom, swim from there to the north, climb up a hill, wait for an event to be done and run half a circle on the second level to get to the POI south.

Withdraw consistently works as one of the few instant heal skills and once mastered its mobility in a fight is a huge asset. Withdraws instant cast mobility is a big reason it is so good. Removing or separating its mobility would effectively gut the skill for many players particularly in sPvP and WvW.
I hate saying L2P but Withdraw in this case really is L2P. This is particularly true since healing is easy to swap out for one works in a specific situation better. The thief already has plenty of solid heals to choose from.
