This would mostly be for Roaming/Small Group WvW, I would have the survivability to Zerg Surf I think, but it looks good, but I would appreciate some advice or tips, ect.

Looks good to me. Personally however I would put a sigil of impact into the hammer, also I would change warrior’s sprint to Signet Mastery. As you’ve got a warhorn, you’ll have perma swiftness and you’ll also have higher uptime of boons from Signet Of Rage.

Not sure why you went for Healing Surge I guess you like the adrenaline for the hammer, cleansing Ire grants enough adrenaline though and also the passive healing from Healing Signet when players are being CC’d by hammer makes it the better heal in my opinion. All to your own preferences I guess but definitely would change warrior’s sprint for signet mastery.

If you’re roaming you’re most likely going to run into some kind of class with a condi spec, which will devastate you unless you’ve got someone in your party with party-wide condition removal.
Also, I don’t think you need Melandru runes with the amount of stun breakers you’re running.
