Probably best for you to just let it go and join a different party and try again.
This is kind of a glitch in the reporting system that Anet needs to address. Once you leave an instance (being kicked from a Dungeon party), you no longer can access the party names to report them (clears chat log). This complaint has been standing since the report system come out.
I don’t run Dungeons, but several guild mates told me they take a screen shot of each PUG (or unknown leader) party early in the run so at LEAST you can manually type in the names from the graphics in a report. Definitely a PitA, but can give some satisfaction to an otherwise frustrating situation.
Probably best for you to just let it go and join a different party and try again.
The above solution is most likely the most widely used and may be the main reason the glitch has never been addressed. I would also venture that this may also be a HUGE reason many players try Dungeons a few (or one) time(s) and never return.

Pretty much this. I’m a chronic screencapper too. I first get every account name, and then if there is any conflict in party chat, I screen that. I’ve never been kicked, but I’m too paranoid not to protect myself with the screens, so at the least I can block those who might grief me. (But again this has never happened to me)

I was booted from CM path 1 near the boss on 4/30/14 at around 9:25am. not sure how to report that or it could be reported.

Odd what would you report it as? Who would you report? Who would be held responsible? The one who initiated, the one who seconded, both? Could be easily griefed.
I would argue just do it again. CM p1 is a 10min run. Seems to me to be something that may clog up the wheels of customer support.
I would say report them all and let Support sort it out. I’m guessing they would ignore singular reports, but if the same few guys are doing this to multiple players (whether with any sort of reason or just for mean spirited kicks) someone needs to be made aware of it.
Again, the policy of “that’s just the way Dungeon PUGs are”, is never going to improve the current system. I get that many Dungeon runners don’t think that’s a bad idea and again, that attitude is part of the problem with Dungeons (for non-runner players).
