After complete the story steps needed for the necessary items they do not unlock:

- Golden Whispers Buckle from story step “Strange Observations”
- Waterproof Whispers Padding from story step “Buried Insight”
- Dragonvine Whispers Strap from story step “Heart of Thorns”

Did with two characters already, and do not work. The purchased ones all worked.

Edit: Figured out the problem.
If you start the collection with one character and before complete you jump to an alt the collection does not unlock for that alt, you need to go back to the character that you started with so you can finish the collection. Still a problem thou…

This is still an issue, I have completed Hearts & Minds but I have no idea which character I started the collection on. I was just checking old achievements for ones I could complete, and this stands out. Very frustrating.
