(The items from HOM do not disapear once transmuted… Nothing changes and the gear looks the same.)

you cannot transmute items that you ware wearing any more. try taking them off first.
I transmuted all day long heritage gear from my HoM. Worked like a charm, no problems at all. Just remember to take off the gear like Miotas said.
