I crafted an ascended helm and transmuted it with the exotic one. Everything was fine, I logged out and the day after when i logged in, my ascended helm was not here and was replaced by a white head armor. It’s pretty annoying since i lost the ascended helm and the skin (phoenix armor one)

I recommend submiting a request at Customer Service and tell them all the details how and when it happened.
I recently lost 2 of my items , simply dissapeared in front my eyes when I unequipped/swapped gear .
This should be #1 on anet list to figure out how and why items dissapear.
It worries me that I don’t think they got inventory or somekind of an item log, but they have a policy that they can’t send / recover lost items.
I am still kind of angry and dissapointed that my back item is gone, it had the AntiToxin injector skin, everyone has it but I can’t get it again….lost and I liked it a lot.
