You gain a summoning charge when ever you kill a foe. At 26 charges you summon a fleshreaver. (You lose all charges on down.)
This sigil often doesn’t give you a charge for killing a foe at all. And it seems that there is a very long unknown cool down after the fleshreaver has died, before it starts gaining charges again. This probably isn’t as intended, because this means you during any dungeon run you may summon no more than one pathetic fleshreaver, and spend the rest of the run carrying the item around for no reason.
