I have completed the story mode for the dungeon in question, on the character I’m using. In fact I have beat in on several characters. But I can’t reactivate the rewards track.

What do I need to do to fix this?

Have you completed the Story Mode Dungeon since the Reward Tracks were implemented? It doesn’t grandfather in previously completed Dungeons.

Another quick question. Does doing the dungeon story mode on one toon unlock the dungeon reward path across all characters on your account?

it unlocks for all characters on your account
