But I think I have more details since it’s happened to me multiple times. I am using full set of Superior Rune of the Afflicted.

When I go down, if the Death Nova kills more than one target (seems to be ok with 1) I rally immediately and I have access to all my skills, but I can’t move. I don’t have downed skills. I can teleport to a waypoint and logout (I usually do one of these).

Have the same issue and it’s quite annoying to say the least.
The only thing that breaks this can’t move is going down again and rally in another way OR using my lightning flash.

Verified, I also posted this once, And how typical, I’m ALSO using runes of the afflicted. and the deathnova thing I cannot verify directly, but If enemies die of ANY blleds venom direct dmg at the moment I go down, the stuck thing happens…

/dance will get you out though
