When I completed Hearts and Minds story mission you get an item called Dragonvine (insert order here) Strap, which should be used to improve the Crusader’s Pack Collection you earned at “Prisioners of the dragon”

When I completed for my Vigil character Hearts and Minds the dragonvine strap was gone from my inventory. It was there in the rewards pop up. But not there in my inventory, nor did it unlock in the collection. It unlocked without any problems when I did the previous time for the Agent’s Pack collection. Has this happened to anyone else?
Just the story instance again with a 2nd char and the same bug has happened, no unlock on the collection item.

I found myself similar situation with Agent’s pack. I found this https://wiki.guildwars2.com/wiki/Talk:Scholar's_Pack_%28achievement%29 and did the story with the character which originally unlocked the collection. Then I was able to get items to progress the collection.
