Not the case when you teleport onto stairs as in WvW tower (which almost always fail). The exact way the teleportation works can be very tricky. The distance to the destination may (or may not) be a factor to its successful execution as well. There have been tips and tricks for this skill to help you take shortcuts that are seemingly unreasonable… especially in WvW – like teleporting to a bridge above you that require quite some walking distance.
Also when the terrain is complicated (very uneven terrain), there is no telling the skill will work. Therefore it can be pretty safe to conclude that… the skill is either bugged or not fully polished with definite design flaws.
A suggestion is to make some skills (such as teleportation skills) be more forgiving – rather than fail the skill and go into full cool down, the skill could be locked until a proper destination has been chosen for teleportation (as in World of Warcraft a skill can’t be used until the enemy is in range).
This has been a very long-lived bug.

Yes it is a terrible bug. Makes the skill unreliable: for example it doesn’t even work on EotM bridges.
Bridges in EotM, stairs in WvW (and some bridges), and uneven terrain are where this skill can fail. I have found that in EotM and WvW, the best bet is: 1. If the bridge can be destroyed, 2. If they stairs are on a wall that can be destroyed or upgraded with walls, then the skill will fail. Terrain, your guess is as good as mine as to why it fails sometimes.
It does need polishing and fixing, IMO. Thieves do not encounter this (Or rather, I have not ever encountered it on my thief, but always encounter it on my ele)
This is a well known issue and is related to pathfinding in general. Some areas it benefits you, like teleporting to the bridge in Ogre Watch, and other times it fails to move you a foot in front of you. There is not much they are able to do to fix it short of completely redoing the engine and/or maps. This has been discussed to death.
