Emerald leaper puzzle bug
First thing that comes to mind is that you didn’t do the emerald jumping puzzle. there are 3 borderlands: red, green, blue. your server is one of those (whichever color your’s is on the map).
red = garnet ach
green = emerald ach
blue = sapphire ach
the key isn’t consumed until you open the big chest. any key can be used to open the chest on any of the 3 borderlands. (you can do the puzzle on red, then go to green and open the chest)

Take a look at the screenshot – I’m standing in the vault near the big stash and both keys are in the inventory. The same was done on the other character. I performed Emerald, because this week Blacktide is a “green server”. I see no reason to torture myself with other color jumping puzzles without achievement completed.
And I’ve already opened the big chest, but keys still are in my backpack. Those arrow carts are from that chest. Also I relogged and fully restarted the game – keys didn’t disappear.
If no official answer is received, probably I’ll try to open a vault on another borderlands. Thanks for the idea.
I’ve had this happen, too. I had to go back and open the vault and chest alone. Then I got the achievement.
After relogging and getting back I see chest already opened and I can’t open it again. I’ve completed the puzzle on both my characters and nothing changed…
isn’t there a once-per-reset timer on those chests? I think they added that so people would stop turning the gift of battle into the gift of jumping.
anyways, if you had 2 sets of keys, it would only take one set when you opened the chest. or maybe it bugged and didn’t take your keys and didn’t give you credit. either way, go try it again after the daily reset and see if it works (you just need to open the door and chest – not the rest of the puzzle)

Well, thanks everyone who was trying to help me with my issue, finally I’ve managed to unlock the meta achievement (without puzzles it was nearly impossible – only one week left and I can’t play too much). Some tinkering and great help from Hinote made it through.
