My Fractal Harpoon Skin is not unlocking in the achievement panel. I even have the skin equipped and took it out of storage and tried to apply it to a weapon and still wont unlock.

In particular, any skin that was originally unlocked in the wardrobe might not have been added to the collection when HoT launched and currently, there is nothing you can do in-game to change that. ANet has reported that they are aware of the issue and working on a fix. A lot of us believe (or at least hope) that it will be fixed with the fractal patch mentioned by brunobyof, promised for this month (presumably along with the Wintersday update on the 15th).
