The hero challenge in Spider Nest Cavern (Diessa Plateau) does not display on the map, on any of the levels you can select. It also does not show up on the mini map even when in range.

Yeah there’s a couple like this. Please fix these ArenaNet! (As well as re-paint the icon blue! )

One I know of is the Harpy Nest area in Fields of Ruin. I actually finished the Hero Point Challenge there before I realised it didn’t show until I completed it!

As far as I can tell, it happens at specific challenges (I’ve seen it at this one, and the one with the harpies in Fields of Ruin), but only when you first uncover the section of map in which it should appear. Leaving the map or logging out and back will then cause it to appear normally.
