Holding a boulder at end of boss fight in Ascalonian Catacombs causes missing chest. We have seen this bug occur on multiple accounts in our guild runs in AC story mode.
If you have a boulder in your hands when the boss dies, you will not get a chest. Other people in your party will. The chest will show on the map, but wont be shown to the person who had the boulder.
