This is an old bug too. I’m only experiencing it on my warrior, even when doing nothing but standing round. Full HP then dead.
It even occurred once in an uncontested outpost. I was logged out of character, logged back in dead.
Spoken to others who are also experiencing the same bug…again.
Ooops, reads downed skill ‘vengeance’. *facepalm….
At least some of the time that must have been a cause :P
