Loosing target
Same happened to me. While keeping right mouse pushed down, i targeted the 1st friendly target my mouse hovered over. Can be fixed though by disabling “Richt-Click to Attack/Interact” (Options -> General Options -> Combat/Movement)
After last major patch i keep loosing enemy targets and get a random friendly target instead. This is happening when i try to move my camera (mouse right click pressed) without actually clicking on any other friendly player. If there is no other friendly player around, my target just deselect randomly.
Worked very well before, and by talking with other players seems a lots have same problem, but i didn’t found any thread about this.

Hi. All of these are related to the same issue.
Issue: Mouse Button Loses Focus and Does Not Reacquire It
Most of the time that this issue is present (it is inconsistent), the mouse actually loses focus and never reacquires it, which prevents character and camera orientation with the right mouse button and leaves the game in a completely unplayable state. Some days it is very bad and prevents you from even being able to play, other times it is only a couple times a minutes, but leads to frustration and not playing the game the rest of that day.
Issue: Nearby Friendly Target Being Selected Unintentionally Due To Mouse Button Losing Focus and Reacquiring It Quickly
The recently added option to ‘Disable Right-click Targeting’ has helped with the linked issue of randomly selecting a nearby friendly target while moving your camera with the right mouse button.
This happens because the mouse tracking has an issue to where the game reads you as releasing the mouse button (despite your having never released it) and then re-‘clicks’ the button down quickly. Due to this issue, if your cursor was over/near another friendly, then they become your new target as if you had intentionally clicked on them to select them.
Issue: Total Target Loss Due To Mouse Button Losing Focus and Reacquiring It Quickly Over Empty Area of the Screen
However, disabling right-click targeting and interaction has simply made the issue of a new target stop happening, the issue of losing your target due to the glitch in mouse focus persists. Now we just have target loss due to the quick un-click and re-click of the right mouse button over an empty area of the screen which deselects the current target; instead of both target loss and random friendly selection.

Best i can tell, it is a client workload issue. more likely to happen when there are multiple NPCs/mobs milling about to stress the client into miscalculating the timing between a click and a hold.
really wish this would get fixed, In massive zerg fights like the new Queen’s jubilee I find myself targeting 10 different things before I’m back on the boss

I turned that off and still get the issue. That’s not the problem. Right clicking will make it think you clicked on whatever your mouse cursor was on and instead will select that even with that option off.
Posting here real quick to add another ‘area’ of target loss, and I think this is my issue after disabling the right clicking (I actually think I have not only the right click issue, as some days are worse than others, but my mouse is actually acting up as well so double the fun).
If you have a target, and LEFT CLICK somewhere on the screen off of them, you lose the target.
I think what’s happening is when I’m casting something on the ground (I have the default actions for that), and I don’t think it goes off for some reason, I get click happy and start trying to spam the action (which then causes me to click on another target).

..and here’s some more thoughts. I notice sometimes that I use the left button sometimes to walk forward while I have the right button held down to move the camera. I suspect the target loss in that situation is the right click failing, and the system going “oh, you’re left clicking on that empty spot? okay…”
