I might be wrong, i have had this issue also, but, this has only happened when I have been lazy (or too laggy), ignored all the risen and just lurked by the champ spider spawn location to get the boxes from her, once i had put thought into this, made sure I participated in all the waves, issue seems to have gone. Hope this helps.

Dunno…. Thought I had to tag the priest to get it, but then I’m usually working also to see that the event cannot fail, which has me tagging most spawns of the defend event.
