Nightmare Euthanizer achieve bugged
Same here. Followed dulfy’s guide and it didn’t count. I received ‘Nightmare Ender’ for the first completion, but nothing after the 2nd run.

Completed the tower instance twice by using the correct solutions to the puzzles, and the Nightmare Euthanizer achievement did not complete either time.
