I’ve been using my Proofs of Heroics to buy Notarized Scrolls of Heroics and complete Hero Challenges in Central Tyria on all my characters. Five of them have completed them all without issue; four of them are now stuck on 188/189 Heroic Challenges complete and the vendor will no longer sell them Notarized Scrolls of Heroics. The uncompleted Hero Challenges are in different zones on each of the characters.

Might it be the Home Instance Hero Point? Can’t complete it if it isn’t available, perhaps.

Each of the uncompleted Hero Challenges is in a different zone. On one character it’s in Mount Maelstrom; on another it’s in Cursed Shore; for the third one it’s in Lornar’s Pass; and for the fourth one it’s in Malchor’s Leap. I haven’t looked to see exactly which one it is in those zones, but those are the ones with one uncompleted Hero Challenge.

Try visiting the zone with the relevant character (can just enter/exit or, enter, grab some nodes to pay for WP cost, then exit).

I haven’t heard about issues with Orr map completion, but I do know that when the game is ‘confused’ about a character’s progress, entering the affected zone nearly always resolves things. (It’s more common when ANet has updated the location or internal IDs for a particular aspect of map completion.)

At worst, you could just go and complete the one Hero Challenge manually, although of course it’s better if ANet learns about any bug and fixes it.
