I was doing my Personal Story,
Quest around lv 48. You had to defend the Kral with the Oger against their former leader and his beast. (big scorpion) brandmarked monsters.

I couldnt accept the Quest reward while i was in the quest instance. I teleported just to see if it works then. (Normal case you can teleport and quest reward animation pops up again).
It did not even show up. So i am missing the reward completly, a weapon (longbow) i could need cause my old is kinda low, and complete exp. There is a Screenshot added on this ticket number 120914-005828.

Something similar happened to me. I could select my choice in the instance, but decided to leave. After I left, the window vanished and my reward (and my large amount of XP) were gone. There seems to be no recognition of this bug as of yet.
