I’m not sure if this is a bug or just bad AI, but in Splintered Coast, Sparkfly Fen, there is an escort mission “Escort the quaggans to Quaztocel”. At the time this was happening, Tequatl’s corruption dropped a bunch of Orrian rotmouth landmines on the map. These respawn quickly after being destroyed. Upon reaching the shore, one Quaggan aggroed to the nearest landmine and ran into it, dying and causing the escort mission to instant-fail.
