I just made this video hitting the golem in sPvP to illustrate the bug after noticing a couple of unlucky players get hit by it while I played a match.
Revenant hammer skill 2 (Coalescence of Ruin) is designed to have 3 impact zones with each zone dealing damage proportional to the range from the revenant. In concept this is a good skill design. In practice, it is possible for an unlucky target to receive damage from not one but two of these impact zones with a single cast of the skill.
Normally, the final hit with a fairly standard berserker-stat setup has a tooltip damage of 2200, and the second impact has a base damage around 1700. A target caught by this bug will instead receive a base damage of 1700 + 2200 = 3900 almost instantly, from a skill with a 2 second cooldown. This tooltip number is comparable to the initial series of strikes from Hundred Blades, all at once, and I cannot imagine this is an intended effect of the skill.
To fix this bug, targets should be flagged when hit such that any additional hits cannot be applied from the same cast.
