This happens sinds the release of the latest patch.

Stealth skills don’t trigger stealth but gives reveal as soon you trigger the skill.
I get the feeling, that reveal comes first and disables the stealth mechanic in progress.
I tested this on thief and mesmer in spvp. And notice in both cases this occurs often.

As soon i press for example the skill decoy, no stealth icon appears but right away the reveal icon. This brings the skill on cd and you get a nice reveal bug as bonus.

Pls look into this issue.

This is probably because you i.e. have projectiles on route to an enemy and you apply stealth before the projectiles reach the enemy
