Bugs like these are frustrating, because they reduce your time and effort to 0, and since I was recording our weekly raid, I have figured out it would be easier to show you, than tell you, so behold:

Please fix these, and also please excuse raid leader’s profanity.

It’s very unlikely that anyone at ANet is going to watch a video that might do a good job of describing the symptoms of a potential bug. (Heck, it’s unlikely that more than a few people on the forums will view it.)

It’s easier for you to show them, but it’s easier for them if you explain in text — the more detail the better. (There’s a sticky at the top of this folder with some tips about reporting bugs, although you probably know most of them.)
