This is repeatable and random:
1. Turn off your equipment headgear
2. Use Endless Princess Doll Tonic
3. Enter water deep enough to transform you to underwater gear
4. Exit water
5. You’re now wearing your headgear and it’s still turned off in equipment tab

A. Without your Outfit turned on, you keep your aquatic breather headgear on either for a couple seconds or indefinitely until you go underwater again.

B. With your outfit turned on:
1. You might keep your outfit on underwater
2. You will probably have your outfit headgear on when you exit water

Additionally, many transformation tonics will keep your body in the world the first time you use them in a map

I have this issue happen all the time. I like using Spicy Pumpkin Cookies.. it makes a transformation and applies a food buff. I then cancel the transformation, but I retain the food buff.. After that if I jump in the water I will randomly get my aquatic headgear showing up on my character from that point on even if they are in an outfit. The only way to resolve this graphical issue is to toggle the visibility of my head armor piece.
