At the last body the achievement is not being given. It allowed me to pick up the item but then nothing. Please see attached screenshot.

Not sure what happens after this, hope it helps, either way, keep up the great work!

There is a bug with that where the explorer become interactable after you already completed that objective.

You are at the wrong body. That is the body for “Since he went on an upper ledge to examine the Exalted portal, the asura researcher hasn’t been heard from.”
