This happens right after a cutscene in a dungeon. The switch weapon button is greyed out. Playing a necro, I usually just go in and out of DS real quick to fix it but I’m told that it can also be fixed by accessing the Hero menu and switching from there. It’s a workaround, yeah, but the problem is easy to miss and can cost precious seconds when already in combat.
