From Wednesday’s patch notes:

BUG-FIXES

• Fixed an issue that would cause downed or defeated players to not get credit for Kiel: Dungeon Runner or Gnashblade: Dungeon Runner achievements.

Were you perhaps downed or defeated when the dungeon was completed? Should be fixed from the patch yesterday.

Did you skip any checkpoints or not actually cross the finish line? The reward chest won’t activate if you skip any, there’s a fairly common skip at the end by Leaping up to the chest and not actually crossing the finish line

Have you completed all 5/5 Daily Achievements? You have to complete a full day to count 1 toward the achievement.

You should check out the Fortune Scraps Vendor in Labyrinthine Cliffs… I think you’ll find what you’re looking for

The Candy Corn/Zhaitaffy merchant is a Zephyrite kid and he is with the other Zephyr Sanctum Supply box merchants at the Bazaar Docks of Labyrinthine Cliffs. You can get there by using the Labyrinthine Cliffs Transport Facilitator in Lion’s Arch

If this happens, look for an Instigator who is in the Downed animation state, yet is still showing the red ‘hostile’ nameplate. Some of the Instigators have a bug where they are like Karka! Meaning, they are damaged down once, they enter the ‘Downed’ state yet remain hostile, then must be taken down again from full health to count toward the event.

For now, to avoid this problem, please close the conversation with the aspect master and choose to join the game from his first conversation screen.

This choice should always send you to the sanctum sprint."
