Not a bug. Achievment comes even if you’re missing some points of interests. All maps will show you 100% but newly added POIs or vistas in patches still have to be completed. I had the same issue.
1. Check every map for updates
2. Make sure you have every sneaky point like the base of the orders of whisperers.
