I noticed a bug in PvE (possibly PvP as well but the timers on when you spawn are different) regarding my warrior dying a second time when I appear at a waypoint. Occasionally I will die and I will choose to spawn back at base. When I choose a waypoint quickly after I die, sometimes I die immediately upon appearing at the waypoint location. This damages armor and requires me to use the waypoint again to respawn.
I’m not entirely sure of all the conditions that create this bug, but it has happened 20+ times throughout my time playing Guild Wars 2.
