Get very close to the door to Cragstead and you will get the area.
Doing the storyline did not give it to me either.

Edit: Cragstead was the one I was missing, that gave me 100%!

I also have all the new areas and checked with http://wiki.guildwars2.com/wiki/User:Bluestone/Explorer for all the areas and still no shiverpeaks explored achievement…( ‘To gain this achievement you need to find a total of 174/174 areas.’ That does not add up…

You are also missing Tribulation caverns. The entrance is near the vista. Behind the veteran elemantal in Dredgehaunt Cliffs
