I have also encountered this bug with the Sapphire leaper achievement. It is the color of my home team and have tried it on two characters; one last night and one this morning. As OP states, the key parts are not consumed when the door is opened, and when you loot the chest you don’t get completion of the achievement. I’ve been able to complete the other two borderlands puzzles and the obsidian sanctum puzzle without any problems.

Sotzz the Scalleywag has been missing for 2 weeks on Sanctum of Rall server. My guild has been doing bounties around reset time on Sunday evenings. 3 to 4 hours prior to that I go scouting for Sotzz looking for him or his “Suspicious Barrels” only to never find either anywhere in the zone. I’ve literally covered the entire canvas of the zone numerous times the past couple of weeks and haven’t found any sign of him anywhere. I’ve checked underwater areas, inside of buildings, inside of all of the caves and tunnels and nooks, in the jumping puzzles, and all over the open areas. I believe he has somehow bugged out and is not re-spawning correctly. I have guested to other servers and found him in less than a half hour, and notice there are suspicious barrels all over the zone. This is not the case on Sanctum of Rall. I have also contacted community members who have submitted spawn point maps on the internet to try to find him. They, also, were unable to find him on SoR by guesting here to look.

Many guilds, including ours, have been forced to fail their guild bounties because they drew Sotzz on this server. I hope this report gives enough headway to hopefully fix whatever is causing him not to spawn before the next update goes live.
