I am trying to request a game refund but the refund page is blank with no options. Please fix it so I can request mine.

I brought the digital version of the game and after playing it for a day have decided its not for me and would like to apply for a refund but on the website the refund request page is empty? Can someone please direct me to a working page or is there a way I can contact the support team by phone or email?
