Edit3: I just looked up who Tybalt is. I thought we had the same issue at first because I didn’t see anyone called Tybalt and assumed that’s what was wrong. In any case, my issue is with the same story.
Yes, I’ve just now come across the same thing. I have Tchkik, Vracken and Sieran following me, there are no other NPCs inside the instance and my objectives list is attached.

Trying it again, there is also Yiktuk, who had to be revived. The first time I did the instance, it marked Vracken with a marker. After talking to him it marked Tchkik. There were no other Skritt that could be revived, and I don’t recall even seeing Yiktuk’s body that time.
On the second try, after talking to Vracken it marked Yiktuk on the map with a circle as it should.

I think the reason my image shows the same objective twice is that after defeating the harpies there was still a Skritt missing (Yiktuk), so instead of giving the reward and ending the instance, it realised there was a completion requirement missing and so it added the second “Rescue Tchkik and the other scouts”. The second time I ran the instance it seemed to have no issues and it did not display that objective twice.
