Never mind, my mouse was physically broken. Typically, it only broke during certain mouse movements. Tried another mouse, and it works.

Ok, I have this issue since the big patch. It’s easy to replicate since it happens EVERY time I do the following:

Right-Click to mouse-look around ( remember where your mouse pointer is on the screen before you right click )
Keep holding the right mouse button down
Mouse the mouse a little, it works (it should !)
Now move to a RED (attackable) mob while mouse-looking around
Now, move your mouse pointer ( you remember where on the screen your pointer is , right ? ) so that it hovers over the mob
Poof, the mousepointer reappears ( red, to indicate a mob is under it ) and you will lose focus as if you depressed your right mouse button. BUT you still have the right mouse button pressed.

Please, disable right-click targetting. That will fix this issue forever. Especially the hovering over mobs which changes the color of the mouse pointer.
