My group and I killed the Toxic Hybrid (1 achievement) and we killed him without dying (another achievement, plus a mini). The game did not register either one of those in the achievement panel.
I just posted a bug report about this yesterday, that has apparently been overlooked. My team and I went in and killed the Hybrid and none of us died doing so and I received neither the achievements nor the mini.
