I have exactly the same problem (since the last lake doric patch) in dungeons , i have report the squad leader like you did.
Its really boring , you do the dungeon and you are kicked of the dungeon because you are now in a squad.
I hope this will be fixed soon. Btw i haven’t take the time to screenshot.
