Some kind of confirmation is a good idea. We have to confirm spending dungeon tokens, and transmutation charges are listed in the wallet as another type of currency, so it fits with the system.
Also, we have to confirm deletion of a lot of silly items. If we have to type out the whole name of a tonic to delete it, we can certainly click a confirm button to transmute.
