My Been There Done That title appears to be stuck at 97% even though I have:
100% completed Vistas, POIs, Waypoints, Skill Points, and Hearts (in PvE and WvW)
Completed all 5 Explorer achievements (Kryta, Orr, Ascalon, Maguuma, and Shiverpeaks)
If someone that knows what else there is to do, or if it is bugged for some people, please tell me.
