If you are joining through the pop-up dialogue or by walking up to the entry point
Tried both

How many people in the party tried creating the instance
Tried with 2, 3 , 4 and 5

Any other details you can provide
Tried everything that I could find in this forum and other GW2-related forum.
Log, relog, change character, restart GW2 client, restart PC, restart modem, invite party, disband party, re-invite again, party leader enter first then invite other to join, everyone has both dungeon option (story/explore), zone in, zone out, ….

Please fix it….

Same problem here, COF dungeon entry is bugged.
Tried all the OPs’ suggestion on how to get in. None of them worked.

It has been 3 days….
