Oh. The helmet does indeed seem to be related. If I hide it on my charr (with the checkbox in the Hero/Equipment window), the weapons are sheathed in the correct position. This is kind of a workaround. Problem is, I do prefer my charr with her helmet on.

I’m having the same problem with my female charr (Guardian). One-handed weapons and some off-hand weapons, when sheathed, appear below the character’s feet, hidden under ground. This has been happening for at least a few weeks. Seems to occur with all weapons of certain types (swords, maces, scepters, torches) not just a particular item.

Note the sword hilt protruding from the ground near the charr’s left foot:
