I had this exact same issue (black screen, music still plays, error code 58:11:5:512:101) and it turned out to be my router’s DHCP lease time, which was set to renew every hour. If this turns out to be the case for you, the easiest fix is to log into your router and change the lease time, either for your specific computer’s IP address or for all devices on the network (all routers are different in this respect). I just bumped mine up from 3600sec (1hr) to 86400 (24hr) but you can obviously do whatever you think is best for your own personal situation.
It is not a true “fix” to the problem, but hey, it is a simple workaround. I never used to have this problem but then it started happening in both GW2 and the original GW. It could have been an update whilst I was away, but it was most likely from me switching over to my dad’s wireless Motorola to use the N-band and I just never noticed before on my old router. I agree it would be nice to have a “grace period” of sorts for the “connection renewal” but hopefully this helps you from getting continually booted as I was!
I really think Dragonfire.6382 might be onto something here. Perhaps this is a defense of sorts against botting or a hacking/duping strategy involving disconnects. I’ll see my wireless connection in Windows 7 momentarily go “yellow” and then it is back again, no problems. However, the connection to GW2 is pretty much fubar as you all know (black screen, music still plays) and no reconnect option outside of Alt+f4 & restart. Personally, I notice the disconnects at certain times (hourly) or during AFK but this could be just anecdotal for me. A lot of people are quick to blame ANet (and, yes, it would certainly be nice to hear from them) but it could just as easily be one of those stealthy Windows updates as well, as Dragon mentioned.
Lastly, I should mention that not only does it happen in GW2 but I have been doing some daily ZM/ZBs back in the original Guild Wars and have been getting code 58’ed there as well. I have been disconnected more in the past week from GW1 than I have in the previous 6-7 years of playing the game (and, no, I am not exaggerating). The “attempt reconnect” has only been 50/50, too, even.
UPDATE (7/20/13):
Okay, so I managed to fix my own problem or at least work around the issue. For my own personal experience, I noticed I was getting disconnected hourly, like clockwork, but at different times everyday. This lead me to investigate my wireless connection and turns out my router’s DHCP lease assignment was… you guessed it, hourly. I just changed it to 24hrs. It is still strange because it never used to happen in GW/GW2, but I guess it was part of an update in the time I took away from the game. I know this won’t help everybody but hopefully it helps a few in my boat!

I definitely agree with all you guys. I think it is only really an issue for people (like myself) who are often have to play alone on my caster. I agree with Samus in that just taking into account the number of people in a certain area and the size of the place should do the trick. The main thing I worry about (speaking to the experience of solo players) is that they fight their way to the back of a crypt for a chest/rich vein only to turn around and see all those monsters they just worked hard to defeat have respawned. So you either have to map to the nearest waypoint or fight them doubly. You could run for it but the combat slowdown makes it risky plus I am mindful about training monsters into another player.
To give a specific example, I was exploring and found the hidden area in the Spider Cave in the Blazeridge Steppes zone. I killed the veteran spider and his spawn but had to kill them 3 times before I found the correct jumping path. Because of the slow down in combat, I mean I had to kill them again to actually make the jump across the rocks, too. I think in this specific example, the vet. spider respawned every sixty seconds.
Do you guys think phantasms/illusions persisting after an enemy dies is necessary?

I agree with a lot said. I especially lament the noticeable lack of aoe in DEs unless you are rolling with a sword or laying down the one and only chaos storm. Shattering is one of our main aoes but good luck trying to get up illusions in any DE with more than three people.
Maybe if they reworked the f1-f4 and decoupled clones & phantasms so that f1 shattered clones, f2 shattered phantasms, and f3 both (they can keep the dmg, confusion, daze) and have one or both types hang around until you dropped combat. Or even if they had a “utility” illusion slot so when you use your main aoe/shatter mechanic you don’t lose a valuable illusion like the Defender along with the disposable clones.
Another thing is most every other class I have played have some nice type of movement buff (signet, warhorn, etc.). The best I’ve seen for mesmers is random swiftness from chaos or signet of inspiration. It would be awesome if they reworked the ‘Compounding Celerity’ trait to do a movement speed or swiftness buff on clone creation or something unique.
I honestly think mesmer will get a little love once the team has dealt with the issues that need real, immediate attention. The mesmer is certainly interesting and unique but a tad underwhelming in pve in my experience.
With my thief, I chose to ignore Quinn but the next follow up is to kill Two Blade Pete anyhow, haha. I remember it being really tough as well.
Another thing, aside from weapon choice and tactics, is to hold off a few levels until you can spec into your traits. I got tired of dying so I invested into the toughness and vit lines (after a respec) and now I think I may have only died one time since (now in my late 30s). It is truly night and day. Also, if you are a jeweler or have a guildie/friend, it can be nice to save a jewelry set exclusively for toughness for big boss/dungeons/story fights (the Ringmaster anyone?
Toughness seems to be one the major component in armor rating, at least early on, in my experience. All the best and keep stealin’!
